Getting health insurance is a requirement of most visas. You usually will have to have sorted it out before arriving in Italy.

Once you are here, if you’re not in the national health system through your job, you have an option to buy into the system either for a flat annual rate (from January to December) for students or as a part of your income if you are on an elective residence visa or other such situations.
